Pain and numbness... now twitching

I had my third back surgery, which was a fusion, six months ago.(L4-5). The pain seems to be getting worse. Ive had very numb legs and that hasn't gone away...I am in therapy and after the last group of approved sessions, I started getting very painful surges of pain at night, then felt a weird feeling in my toes. And to my surprise, my big toe was twitching by itself, Does this mean that my already damaged nerves in my legs are becoming more damaged?
Before my 2nd surgery, I lost complete movement in that foot... that is my fear... could this happen again?
How do I know they operated on the correct spot? If the feeling is the same and getting worse... I did have a tear at L5-S1
I refuse to just get a shot to cover up the back pain...my legs are just as important!
