Narrative
MRI lumbar spine without contrast.
INDICATION: Right lower chimney radiculopathy, low back pain
COMPARISON: None.
TECHNIQUE: MRI lumbar spine was performed utilizing T1, T2 weighted sagittal
and axial imaging. T2 sagittal STIR sequence was also performed.
FINDINGS: There are presumed 5 lumbar-type vertebral segments. The vertebral
body heights are preserved. Anterior endplate degenerative changes
demonstrated at the T12-L1 level. There is mild anterior discogenic spurring
and bulging at this level. There is mild disc desiccation at the L4-L5 level.
The paraspinous soft tissues are unremarkable for edema. The conus position
and signal is preserved.
The L1-L2 disc space height is preserved. The canal and foramina are patent.
The L2-L3 disc space height is preserved. The canal foramina are patent.
L3-L4 disc space height is preserved. The canal foramina are patent.
The L4-L5 disc space height is preserved with disc desiccation. There is facet
degenerative change. The canal and foramina are patent. There is a far right
lateral annular tear adjacent to the exit zone of the neural foramen.
The L5-S1 disc space height and signal is preserved. There is mild ligamentum
flavum prominence. The canal foramina are patent.
The visualized paravertebral soft tissue structures are within normal limits.
IMPRESSION:
1. Lumbar facet degenerative change L4-5 level.
2. Far right lateral annular tear L4-5 level adjacent to the exit zone of the
neural foramen without focal disc protrusion.
3. Anterior endplate degenerative change T12-L1.
4. No focal disc herniation.
Now I am 6 weeks into Physical therapy as directed by the orthopedic and I see him for a follow-up tomorrow but I want to make sure I know exactly what is going on. So is the MRI report saying there is a tear adjacent to the exit zone of a nerve?
My symptoms are right leg soreness/pain. It starts at thigh level usually and runs down leg to almost the top of my foot. Occasionally numbness and the pain is positional . Definitely when I am sitting on a soft surface and after a long day of physical activity. The soreness is a daily thing but somewhat manageable with movement, OTC Motrin and heat at night.
